Sharing Care Agreement

Carers and family members are now able to be part of a patient’s care under the Sharing Care Agreement introduced by Derby Teaching Hospitals.

The agreement is a written document between a carer and ward staff which lays out how a carer would like to be involved with the provision of care. This may involve staying with the patient overnight, supporting at mealtimes or when washing and dressing, or being involved with more specific care tasks.
By signing the document a carer is agreeing to inform staff when they leave the ward or department to ensure continuity of care and supervision.

Please inform patients and carers about the Sharing Care Agreement if they have concerns about the potential disruption to continuity which can be caused by an extended visit to hospital.
