Sharing care agreement

When you come into hospital, your carer or loved one can be involved in the care you receive. This may involve staying with you all the time and providing help with washing and dressing, eating and drinking or being involved with more specific care at certain times of the day.

The Sharing Care Agreement is a signed written document between your carer or loved one and the ward staff to help clarify specifically how your carer/loved one will be helping to provide care. This agreement can be updated as required to enable your carer/loved one to provide more or less care as needed by you.

As part of signing the Sharing Care Agreement your carer or loved one is agreeing to inform staff when they leave the department/ward, to ensure you continue to be provided with comfort and supervision by the ward/department staff in their absence.
