Community palliative care team


The community palliative care team are a multi-professional team of palliative care clinical nurse specialists (Macmillan) and palliative medicine doctors.

We offer advice, support and care to patients with advanced, progressive and life-limiting illnesses, both cancer and non-cancer, their carers and families and staff in the wider Southern Derbyshire community.


The advice and support includes

  • assessment of palliative care needs
  • symptom management
  • psychological, social and spiritual support
  • end of life and terminal care
  • advice regarding basic financial benefits
  • information about other community services

The service is available seven days a week

  • Monday to Friday: 09.00 - 17.00
  • Saturday & Sunday: one Macmillan nurse is on duty 09:00 -17:00 to give advice and urgent visits

Palliative medicine consultant visits can be arranged on request.

We also provide a service to five community hospitals in Southern Derbyshire

  • Babington Hospital 
  • Ilkeston Community Hospital
  • Ripley Hospital
  • St. Oswald's Hospital, Ashbourne
  • London Road Community Hospital


Other Information


Referrals can be made by the primary care team / GP and should be made to the team secretary via telephone.

Patients must be aware of and agreeable to the referral.

In the case of referrals made by sources other than the GP, the GP will be contacted to discuss before the referral proceeds.

Out of hours enquiries should be directed to the Nightingale Macmillan Unit out of hours advice line for patients, carers & health professionals.
