Clinical Pharmacy Service

The Derby Teaching Hospitals clinical pharmacy team  provides a comprehensive range of specialist services to wards and clinics at the Royal Derby Hospital and London Road Community Hospital.

This includes:

  • Providing advice to clinical staff on medicines prescribing, preparation and administration

  • Optimising and monitoring the effectiveness of patient’s medicines

  • Identifying adverse effects of medicines

  • Ensuring patients and their carers receive the right information and advice about medicines during their stay and on discharge

Our team of pharmacists and medicines management technicians work closely with clinical staff on wards to ensure patients receive the right medication at the right time and in the right way.

The pharmacy team will talk to patients after they are admitted to:

  • Get an up-to-date list of which medicines a patient takes

  • Identify if any of their medicines may have contributed to the patient  being in hospital

  • See if the patient still needs all of their medicines and whether the patient experiences any difficulties taking them

  • If problems with taking medicines are identified the team will attempt to help patients solve them with a variety of solutions. 

The team can refer patients to their normal community pharmacy (“Chemist”) to further support them to take their medicines after they are discharged if this is required.  We may  suggest this where a number of changes have been made to a patient’s medicines during their stay in hospital, or where a number of new medicines have been prescribed.

To further improve the quality and safety of medicines use at Derby Teaching Hospitals, the clinical pharmacy team has expanded to include a number of non-medical prescribing roles.  These prescribing pharmacists work in several areas of the hospital such as with the Frail Elderly Assessment Team, on the Short Stay Medical Wards and with our Out -patient Antibiotics Team.

Away from the wards the clinical pharmacy team are also involved in;

  • Writing guidelines to help with treatments for groups of patients

  • Investigating new treatment options and helping decide whether they are appropriate for use within the hospital

  • Conducting education and training sessions for other clinical staff

  • Investigating and reporting errors if they occur and identifying solutions to reduce the risk of them happening again

  • Ensuring the hospital gets the best value from the medicines it uses

  • Linking with colleagues in other hospitals both regionally, nationally and internationally to share best practice
