There are four main ways in which GPs can make a referral:
Urgent care referrals:

A single point of contact for GPs and primary care staff is available to discuss the urgent care needs for patients. We will support you to identify and access the correct service and care plan to meet your patients needs.
Advice from a consultant or registrar is available from 9am-5pm Monday to Friday, for the following specialties:
Gastroenterology
Renal
Cardiology
Medicine for the Elderly
Acute Medicine
Alternatively - if you feel your patient’s needs are less acute, you can also be transferred to a member of the Service Navigation Team from 8am-10pm, seven days a week for support in accessing community health services.
Assessment - your patient can be booked into an assessment unit in the usual way and the Bed Bureau team will make the necessary arrangements.
Admission - arrangements with the Bed Bureau can be made by contacting this number 24 hours a day, seven days a week.
New outpatient referrals:
We aim to make it as easy as possible for those referring to services at Derby Hospitals. In line with national priorities, we ask all those referring from primary care to use Choose and Book wherever possible. Standard referral forms are available to download here.
Please note some specialised services are not available on Choose and Book. Where this is the case, we accept written referrals. Guidance for written referrals can be found here.
Cancer 2 week wait:
All cancer two week wait referrals should be made through Choose and Book. Where there is no slot you should defer to provider within choose and book. We can instantly see patients on our appointment slot issues work list.
Direct access services:
Some services are available for direct referral by a GP. We have a dedicated GP telephone line Tel: 01332 786082.
