Become a member

You don't need any special skills or experience to be a member of our NHS Foundation Trust. You just need to be interested in our hospitals and the work we do.

You must be over 16 years old, although there is no upper age limit for membership. You also need to live within our catchments area, which is defined by the following local authority electoral areas:

  • Derby City

  • Amber Valley

  • Erewash

  • South Derbyshire and Derbyshire Dales

  •  Rest of Midlands (including North West Leicestershire)


If you live outside these areas, you will not be able to become a member of our NHS Foundation Trust. However, there may be a Foundation Trust near you where you can become a member. If you are a member of another NHS Foundation Trust, you can still become a member of ours. Our staff are automatically members of the NHS Foundation Trust unless they choose to opt out.

Confirming your membership

When we receive your completed membership application form, we will welcome you as a member by sending a confirmation letter and membership pack. This pack will include a membership number. It is important that you keep this number safe, as it will help us to answer any queries you have more quickly.

Opting out

If you don't wish to be a member any more, you can terminate your membership by contacting us in writing:
