Over a thousand diabetes patients screened as Derby partnership is shortlisted for two health awards

A partnership between Derby Hospitals NHS Foundation Trust and a group of local Derby GPs has been shortlisted for two awards.

'First Diabetes’ is a finalist in the Health Service Journal Awards 2012 in the ‘Managing Long Term Conditions’ category and has also been shortlisted for a Health Enterprise East 2012 Innovation award in the category ‘Disease Prevention, Screening and Diagnosis’.

The First Diabetes partnership between Derby Hospitals and local GPs provides care for over 2,500 patients in Derby with diabetes. By providing routine diabetes care in the community, patients no longer need to visit hospital for treatment. Having a shared patient record gives all clinical professionals involved in the patient’s care access to the same information, therefore eliminating duplication andensuring that the patient receives a consistently high standard of care. Patients also receive examinations for common issues such as eye and foot problems as well as regular check-ups and blood tests.

‘First Diabetes’ has screened over 1,000 adult patients for diabetes with 15 new diagnoses made and over 200 patients attending group education in the last three years. Within the first year, 91 patients who had been referred for better blood sugar control had lowered their levels and 39 patients had achieved a 1% or greater improvement.

Dr Rustam Rea, consultant physician and co-chair of First Diabetes Clinical Board, said: "On behalf of the ‘First Diabetes’ team we are delighted to have been shortlisted for these prestigious awards. The judges have recognised our innovative work in bringing the hospital and GPs together to help patients with diabetes manage their condition more effectively. This has led to better care for our patients and the opportunity to be seen closer to home."

The winners of the Health Enterprise East 2012 Innovation awards will be announced on 10 October, and the HSJ Awards 2012 will be announced on 20 November at the Grosvenor House Hotel in London.
