Derby’s pioneering diabetes care wins national award

A pioneering joint venture, which benefits diabetes patients in Derby, has won a prestigious national healthcare award.
Intercare Health, a partnership between Derby Hospitals and the city’s GPs, has won the diabetes care award at the Patient Safety and Care Integration Awards.

Intercare Health is a different model of care in Derby, allowing diabetic patients to see specialist doctors, nurses, dieticians and other support teams all under one roof, without the need for repeat hospital visits. Bridging the gap between home and hospital improves care for patients managing their life-long condition.
“We’ve worked very hard as team here in Derby to introduce this new model of care for 15,000 people with diabetes, providing care in a different way that’s really making a difference and which has had such positive feedback from GPs, from nurses and other health professionals and from the patients themselves. To have this recognised nationally is fantastic for everyone involved” says Dr Paru King, the Consultant Physician who is Intercare’s Clinical Lead.  
“It is great that this innovative approach to working differently, putting the patient at the centre of their care, has received national recognition. We’re also pleased to see the focus we place on patient safety and experience, which has extended throughout the Trust, being recognised nationally in the commendations we also received” says Derby Hospitals Director of Strategy and Partnership Dianne Prescott.
At the Patient Safety and Care Integration Awards ceremony Intercare Health was also Highly Commended for its Proceed project, offering pre-conception advice for women with diabetes who are planning a pregnancy.

Derby Hospitals NHS Foundation Trust was Highly Commended for its pioneering work in setting up the UK’s first electronic monitoring system to identify patients with life threatening Acute Kidney Injury.

Overall, Derby Hospitals were shortlisted finalists in five of the award categories.
