How to refer patients

In line with NICE criteria we accept patients with BMI =40 or BMI =35 with obesity-related comorbidities (type II diabetes mellitus, sleep apnoea or obesity hypoventillation syndrome, ischaemic heart disease, hypertension, polycystic ovary syndrome (PCOS), hypercholesterolaemia, fatty liver disease, arthritis).

We do not accept referrals for patients who are presently smoking.

Referrals should be made by the patient's GP by means of a letter that states current and highest BMI, present co-morbidities and medications, previous weight-loss attempts and confirm non-smoking status. Please note the previous obesity referral GP proforma is no longer needed. We are unfortunately unable to accept consultant to consultant referrals. We can accept referrals directly from Tier 3 providers. The current NHS England guidelines stipulate that all new referrals should have completed a Tier 3 weight management programme.

Please send referrals to the following address:

Bariatric Co-ordinator, The East-Midlands Bariatric & Metabolic Institute (EMBMI),

Level 3, Royal Derby Hospital, Uttoxeter Road, Derby DE22 3NE.
