About

The cardiac rehabilitation service provides information advice and support to patients with heart disease. To help patients and their carers understand their condition and encouraged to achieve their full potential in terms of physical and psychological health and  adjust their lifestyle.

Our cardiac rehabilitation programme aims to help you understand your condition, to prevent your condition from worsening, and to reduce your risk of future heart problems. These can add up to a better quality of life.
Cardiac rehabilitation helps you rebuild your life, both physically and emotionally. It may seem like a lot of time and effort. But as you get stronger and learn how to manage your condition, you are more likely to return to a normal routine and enjoy life more.

Patient with the following conditions can be referred to the cardiac rehabilitation department

  • Myocardial infarction (heart attack)
  • Coronary artery bypass surgery
  • Heart valve surgery
  • Angina
  • Coronary angioplasty and stent
  • Heart failure
  • Implantable defibrillator

 

Inpatient

Phase 1
A member of the cardiac rehabilitation team will visit the ward to provide individual patients with information about their condition and  treatment ,giving individuals time to discuss any concerns and identify personal lifestyle goals. Discuss a graduated activity plan, work and occupational guidelines

Outpatient 

Phase 2Early post discharge
After discharge from hospital a cardiac rehabilitation specialist nurse  will contact you by telephone offering the opportunity to discuss your progress and discuss any questions you may have about your condition.
You will be invited to attend the cardiac rehabilitation assessment clinic This is an chance for the patient to have an assessment of their individual physical and psychological needs so that a plan of care is developed to promote recovery and maintain a healthy lifestyle. This will include guidance on dietary needs, exercise, medication, stress management and relaxation.
If you are unable to attend  the assessment clinic a home visit may be offered
You will be given the chance to choose from a menu of phase 3 education and exercise programmes that suit your individual need.
 

Phase 3 Rehabilitation Programme
The cardiac rehabilitation programmes is run by a multi-disciplinary team including cardiac rehabilitation nurses, dietician, physiotherapist, qualified BACR exercise instructors and primary mental health advisors who will monitor you closely during these sessions.

 If you have decided to attend one of our rolling phase 3 group exercise education programmes you will be given a date to attend. On your first session you will be introduced to the group, and asked to undertake a shuttle walk assessment and which enables us to provide you with an individualised exercise plan and gym induction

We invite you to attend for the combined exercise  and education programme each programme runs twice a week for seven weeks

.Alternatively you can choose to attend either the education or the exercise sessions as standalone options

Attending the cardiac rehabilitation programme will assist you to:
  • Exercise at a level that improves cardiovascular function
  • Know the parameters of exercise in which to work safely
  • Gain confidence and improve quality of life
  • Have the opportunity to discuss any concerns
  • Gain support from others taking part in the programme

Home Based Programme
If you have chosen a home based programme you will receive a home exercise booklet in which your agreed lifestyle and exercise goals are recorded. A personalised exercise programme and education will be established, progress and support will continue via telephone support. On completion of the 7 week programme you will be invited to  re-attend the outpatient clinic to discuss your progress and plan your long term support.

Seated Exercise Programme
For those with physical limitations, a weekly seated exercise and education programme is run at the Royal Derby Hospital.
